MCPHD SMILE MOBILE AT SENSE MONDAY, NOVEMBER 6TH & THURSDAY, NOVEMBER 9TH
It is that time of year again for our students to receive dental services! This year the Marion County Public Health Dept. Smile Mobile will be providing the dental exams, x-rays, cleanings, fluoride and dental education for all students who turn in a completed form on the day of the Smile Mobile visit. The MCPHD Smile Mobile accepts Medicaid and Personal Insurance as payment. If those are not options for you Smile Mobile will send an envelope home with your student so you can mail the $20 administration fee in at a later date. No money is due on the day of services!
Your student (or yourself) must pick-up a form from the front office if you are interested in your student receiving dental services from the MCPHD Smile Mobile on Monday, November 6th and Thursday, November 9th. Forms must be filled out completely and returned by the day of the visit.
