The MCPHD SMILE MOBILE is visiting SENSE next Tuesday, 9/22
The Dentist will be seeing students who turn in a completed dental form by Friday, 9/18.
The Dentist will be seeing students who turn in a completed dental form by Friday, 9/18.
Forms can be picked up in the front office or by asking your student's teacher to send one home. The Smile Mobile Dental team accepts Medicaid and personal insurance plans. If your student has neither coverage available, they can still be seen! They will be sent home with a self-addressed, blue envelope with a request to mail in a $20 payment.
We will schedule more visits throughout the year with the Smile Mobile based on the amount of completed forms are received!
